Insurance Information
At R. Cole Lambert, DMD, PSC we make every effort to provide you with the finest care and the most convenient financial options. To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. If you have any problems or questions, please ask our staff. They are well informed and up-to-date. They can be reached by phone at Hopkinsville Office Phone Number 270-885-0165.
Please bring your insurance information with you to the consultation so we can expedite reimbursement.
We will be happy to file your insurance claims as a courtesy to you. We will file to any insurance company, but please keep in mind we may not be a participating provider or in network with all of them. Please also realize your insurance policy is a contract between you and your insurance company and you are ultimately responsible for your own insurance benefits.
After you and Dr. Lambert develop a treatment plan, a cost estimate for the planned treatment will be prepared. We will then contact your insurance company and find out what they estimate they will pay on your surgery. We will then contact you and inform you of their estimate. On the day of surgery we will have you pay the difference between our fees and your insurance company’s estimated benefits, and file your claim with them. Most insurance companies pay within four to six weeks. If your insurance company pays what they estimated, your account is paid in full. If they pay less than their estimate, we will send a bill to you. If they pay more than they estimated, we will send a refund check to you.
If you want your surgery to be done on the same day as your consult, please give your insurance information to us at least two working days before surgery. This will allow us time to get an estimate from your insurance company. If you are unable get it to us ahead of time, we will attempt to get an estimate from them on the day of your consult and surgery. If they don’t respond in a timely manner, we will ask you pay for the surgery in full, and refund to you whatever the insurance company pays.
Please call if you have any questions or concerns regarding your initial visit.
